The first session is a time to meet each other, discuss your concerns and determine whether we're a good fit.
While this first session typically includes many questions to understand your concerns and situations, we also review the necessary paperwork and any questions you may have. By filling out the paperwork ahead of time, we can spend most of the session talking about your concerns and current situation. For children, the first appointment is typically only with the parents.
Complete the Intake Forms via our Client Portal
If you are a new client, you will receive a Welcome Email from our Client Portal after scheduling the intake. This email will provide you with your private link and temporary password to the secure Client Portal to sign important documents and fill out necessary information. Please be sure to fill out the forms 72 hours in advance of the appointment and be as thorough as possible. Appointments will be rescheduled if these forms are not completed ahead of time.
Important: Please email photos of your insurance card (front and back) to the office at email@example.com in order to verify your insurance benefits for you prior to your intake session. This is so we can provide you with the best estimate of your treatment costs.
If this is your first time logging in to your Client Portal, refer to your Welcome Email to obtain the temporary password.
Complete the necessary documentation and view your scheduled appointments via the Client Portal.
